Documents requirement for US Visa
1. Current passport with at least 1 blank page and further validity of at least 6 months beyond the intended period of stay in the U.S. (unless exempt by country-specific agreements).
2. Passport containing the most recently issued U.S. visa (if applicable).
3. One colour photo, 5 x 5 cm (2 x 2 inches) and taken within the last 6 months. You will be asked to upload a photograph meeting the Department of State’s photo requirements when you complete Form DS-160. We recommend that you also bring a photograph with you to your visa interview so that it is available if the consular officer asks to review it. It can be the same photograph that you uploaded to the application form as long as it meets the Department of State’s requirements.

5. UK visa/residence permit (if applicable) - it is in the applicant`s best interests to be resident in the UK if applying from here.
6. Non-immigrant Visa Application, Form DS-160 confirmation page.
7. Confirmation page of the application form DS-160 – You must show a printout of your DS-160 confirmation page (the page with the barcode) when you arrive at the Embassy;
8. Applicants who have a medical ineligibility, or who have been denied entry, deported or overstayed must provide additional documents at the time of their application
9. If you have ever been arrested, convicted and/or cautioned (no matter when, where or why this happened) You are required to declare it and bring additional documents with you to your visa interview. The Rehabilitation of Offenders Act does not apply to U.S. visa law
